Using angsizing here, to find the height of Tsuna.
I know for a fact that you cannot see the firt line, but the program I use al;ready tells me that it's 7px.

Tsuna is 1.57 m.
Plugging these values into the angsize calculator and you get a height of 101.54m.
Considering the whole island is what Tsuna sees, and is what takes up the page I will go ahead and assume that it is the same as the horizon distance.
Using the Horizon formula we get a distance of 36.5544 km.
The 429 is the actual forward length.
So since that would be the distance that Tsuna sees that would be 429px.
